 BROOD Shelter Volunteers - You must answer the questions on the adoptability profile for committee review to determine if the shelter dog you are visiting can be transferred to BROOD. Once a new dog has been accepted into BROOD, use the dog intake report to provide basic information. This form is to be used ONLY as the initial report on a dog within its first few days in BROOD. Other more detailed information about the dog is to be provided using the foster dog report form. |
 BROOD Foster Homes - Please use this online foster dog report form to provide or update dog information. When the report is completed, please click the SUBMIT button to see a confirmation page that displays all the information you just entered. You might want to print it for your records. In addition, the form automatically sends an email to the appropriate BROOD coordinators. |
 BROOD Volunteers - Please print the house visit form and the house visit guidelines (pdf requires Adobe Acrobat Reader) and take them with you to use when you do a house visit for a potential adopter. Also print a copy of the pdf disclosure form, the pdf sample contract and pdf child waiver (if appropriate) and leave them with the applicants. Once the visit is complete, please use the online house visit form to report the results and be sure to click the SUBMIT button for a confirmation page that shows all the information you just provided. You might want to print it for your records. The form automatically generates an email to the appropriate BROOD coordinators. |

 BROOD Volunteers - For Owner Relinquishments (ORD's), please print this dog relinquishment form (pdf requires Adobe Acrobat Reader) and have the previous owner sign it when the dog is turned over to BROOD. This form is to be used ONLY for Owner Relinquishments, and it is important that the owner understand that it is a contract between him/her and BROOD. |
